Friedreich ataxia diagnostic odyssey

The work that won a Gold Pharma Lion built the room. This is the operating layer underneath it.

Closed-room neurologist advisory board · AAN week 2026 · Chicago, McCormick Place (April 18–22)

The room

The Sunday-night advisory dinner ahead of AAN — twelve pediatric and adult neurologists around one table, three patient advocates listening, the 21GRAMS team holding the agenda and a single Avant Healthcare moderator running the captures under standard advisory-board conduct.

The pathway

Friedreich ataxia is a rare progressive neurodegenerative disease whose diagnostic odyssey averages five years. Biogen launched Skyclarys in 2023 as the first approved disease-modifying therapy. The 'Friedreich's Back' campaign — Gold Pharma Lion at Cannes 2025 — won the awareness war; the harder war is the prescribing readiness war. This advisory board exists to answer the question Real Chemistry's social analytics cannot: what do the neurologists at the table actually need from Biogen medical affairs to feel confident initiating Skyclarys in the patients sitting in their clinic Monday morning?

Stages of the pathway

Where the room got stuck. What unstuck it.

Each stage carries the barrier, the language that worked in the room, and the team in the client that should own follow-up.

Stage

01

Step

Symptom recognition (pediatric, often misdiagnosed as cerebellar ataxia or clumsiness)

Barrier · Average diagnostic delay 5 years; first specialist usually orthopedist for gait issues, not neurology.

Language that worked

"'Family history of cardiomyopathy under age 40' is the single phrase that triggers genetic testing in the rooms we observed."

Routes to

Biogen Medical Affairs Lead, Rare Neurology — pediatric pathway one-pager (post-MLR review)

Stage

02

Step

Confirmatory FXN genetic testing + Biogen REACH enrollment

Barrier · FXN gene panels are not universally reimbursed; many neurologists order out-of-pocket or skip. After confirmation, Skyclarys prescribing requires Biogen REACH enrollment — an additional friction layer many community adult neurologists are not familiar with.

Language that worked

"'No-cost genetic testing through Invitae's neurology program, paired with the REACH enrollment one-pager' was the combined ask that opened three of the twelve neurologists at the table."

Routes to

Biogen Patient Services — testing-access partnership + REACH simplification (post-MLR review)

Stage

03

Step

Baseline labs + LFT/BNP monitoring counseling

Barrier · Skyclarys initiation requires baseline LFT (hepatotoxicity risk) and BNP (cardiac considerations) plus ongoing monitoring. Community adult neurologists do not routinely run BNP and find the monitoring cadence unclear.

Language that worked

"'A single-page monitoring schedule referenceable in the clinic flow' is what unblocked the room's adult-neurology contingent."

Routes to

Biogen Medical Affairs — monitoring playbook + adult-neurologist peer-to-peer series (Avant Healthcare; post-MLR)

Stage

04

Step

Pediatric-to-adult transition of care

Barrier · Pediatric neurologists feel comfortable; adult neurologists feel under-trained on rare-disease initiation. Note: pediatric label extension is currently in Phase 3 (initiated 2024) — not yet approved.

Language that worked

"'Co-management protocol with a Friedreich center of excellence' was the unblocking phrase. The room asked for a referenceable list of CoEs."

Routes to

Biogen Field Medical — CoE list + transition playbook (post-MLR review)

Stage

05

Step

Caregiver burden + reimbursement navigation

Barrier · Skyclarys is high-cost; patient assistance programs exist but are not consistently surfaced at point of prescription. Reimbursement support exists through Biogen Patient Services but the clinic-flow handoff is fragmented.

Language that worked

"'Single-page reimbursement cheat sheet for the clinic flow' is what every neurologist asked for at least once."

Routes to

Biogen Patient Services — clinic-flow reimbursement one-pager (post-MLR review)

Stakeholders in the room

Who said what, who owns what next.

Pediatric neurologist (CoE)

clinician

Pain · Sees enough Friedreich cases to be confident, frustrated by adult-neurology handoff.

Contribution · Holds the diagnostic-trigger language the room needs to standardize.

Adult neurologist (community)

clinician

Pain · Sees one Friedreich case every 18 months; uncomfortable initiating Skyclarys without a senior signoff.

Contribution · Surfaces the real friction in rare-disease prescribing.

MSL lead (Biogen)

client

Pain · Cannot accelerate confidence faster than peer-to-peer trust forms.

Contribution · Owns the resource gaps the neurologists are calling out.

Patient advocate (Friedreich's Ataxia Research Alliance)

advocate

Pain · Patients are caught between specialties; the lived experience does not match the clinical pathway.

Contribution · Holds the testing-access stories that change the room's mind.

Biogen brand director

client

Pain · Awareness metrics look great; prescribing initiation is slower than forecast.

Contribution · Needs the pathway-to-prescription map this convening produces.

Biogen medical affairs (compliance gate)

client

Pain · Every promotional misstep on a rare-disease launch is regulatory exposure.

Contribution · Must approve every follow-up artifact before it leaves the room.

Question for the client

"Ask Biogen Medical Affairs (Rare Neurology) which of the five resource gaps the room surfaced — pediatric one-pager, REACH simplification, monitoring playbook, CoE list, reimbursement cheat sheet — can clear MLR review and reach neurologists' desks by end of Q2 (Q3 if MLR takes the full six weeks). Real Chemistry × Avant Healthcare has the production capacity for all five."

Follow-up queue · routable now

By Friday, on the right desk.

Biogen Medical Affairs Lead, Rare Neurology

Brief drafted Friday; MLR review begins next Monday (typical 2–6 week cycle).

Three neurologists at the table asked specifically for the pediatric pathway one-pager. Insight logged in the Veeva Vault Medical layer; this Troviii surface is the live capture beneath it.

"Pediatric neurologist (CoE site): 'If I had a one-page genetic-testing trigger sheet I could keep in my clinic bag, I'd test five more kids a year.'"

Biogen Patient Services · partnership lead

Synthesis deck Wednesday next week; partnership conversation post-MLR.

Invitae no-cost neurology program was the single biggest unlocker in the room. The REACH enrollment overlay is the second. Real Chemistry has the relationship to broker a combined partnership.

"Adult neurologist (community): 'I would test every wobbly six-year-old I see if I knew the lab was covered and the REACH enrollment wasn't a separate phone call.'"

Biogen Field Medical · CoE network lead

Veeva insights matrix posted end of month; CoE playbook through MLR Q3.

Adult neurologists do not have a referenceable list of Friedreich CoEs in their region. Five of twelve mentioned this.

"Adult neurologist (community): 'I send to the closest academic neurology center and hope they have a Friedreich program. I should not have to hope.'"

21GRAMS creative · Friedreich extension brief

Briefing meeting next quarter, week 4; deliverables post-MLR review.

Reimbursement-at-point-of-prescription one-pager + LFT/BNP monitoring schedule. Medical-affairs reviewable. Briefing scoped for next quarter.

"Patient advocate Cauley (FARA, on record at Cannes 2025): 'The financial conversation breaks more families than the diagnosis.'"
